Milestone M24 Achieved – Self-management discharge protocol for COPD patients (pilot intervention)

Let’s celebrate together: the JARED project has just reached Milestone MS24, a significant step in its mission to combat chronic respiratory diseases across Europe!

JARED’s Coordination Team is pleased to announce the successful delivery of MS24, a report presenting the initiation of a pilot intervention focused on structured discharge care and digital self-management support for COPD patients hospitalized with acute exacerbations.

This milestone marks an important transition from preparation to implementation. It demonstrates how structured, multidisciplinary discharge pathways can be integrated into routine clinical practice, supported by educational tools and patient-centered care approaches.

Above all, the report highlights the importance of comprehensive support provided for COPD patients during the vulnerable period following hospital discharge. By combining discharge planning, patient education, respiratory rehabilitation, smoking cessation support, nutritional counselling, and digital self-management tools, the intervention aims to improve quality of life, strengthen self-management skills, and reduce future exacerbations.

According to MS24, the pilot study is currently being implemented at two Hungarian clinical centers, and early operational experience suggests that the intervention can be integrated into routine clinical practice.

Furthermore, the report underlines the growing importance of digital health solutions in chronic respiratory care. Empowering patients with accessible information, self-management resources, and continuous support can play a key role in improving long-term outcomes and strengthening more sustainable healthcare pathways across Europe.

Milestone M24 demonstrates that innovation in respiratory healthcare is not limited to new treatments alone — it also depends on better care coordination, patient empowerment, and integrated support systems. Through this pilot intervention, JARED is contributing to a more connected and patient-centered approach to COPD care in Europe.
